The Chronic Disease Self-Management Workshop Program (CDSMP) is a 6-week group workshop series that will teach participants how to live with their chronic condition, including falls prevention, healthy eating, and working better with their healthcare providers.
An informational session will be held June 3 at 1 p.m. at the Forest Hills Heights Community Center. Classes will begin the following week. Interested residents can register online.
